Application
Application forms for the 2012 session of the Summer School on Medicines must be received by June 20th, 2012.
There are a limited number of places for this program. It is incumbent upon each applicant to ensure that his/her file is complete. Applications which are incomplete or submitted after the deadline will not be considered.
Payment
Registration fee:
- FULL SSM4 PROGRAM* : CDN $1,200 (before taxes if applicable) An invoice will be issued upon reception of your registration. Program fees must be paid in full before admission to SSM4 (check payable to Montréal InVivo).
New registration features to SSM4!
- SSM4 5-DAY PASS** : Any 5 days of program in Montreal CDN $700 (before taxes if applicable) An invoice will be issued upon reception of your registration. Program fees must be paid in full before admission to SSM4 (check payable to Montréal InVivo).
- SSM4 3-DAY PASS** : Any 3 days of program in Montreal CDN $500 (before taxes if applicable) An invoice will be issued upon reception of your registration. Program fees must be paid in full before admission to SSM4 (check payable to Montréal InVivo).
* Registration fee includes: Presentation Materials, Luncheons, Transportation to and from Québec city, Accomodation in Québec Cityand Refreshments. Accomodation arrangements should be done directly by participants and payment is separate.

FELLOWSHIP OPPORTUNITIES:
A limited number of fellowships covering at least 50% of the registration fees are available on a first arrived, first served basis, as offered by :
GRUM (Groupe de recherche universitaire sur le médicament):
50% of the registration fees; for application of Université de Montréal
RQRM (Réseau québécois de recherche sur le médicament) :
50% of registration fees for applicant based in the greater Montreal area, or
75% of registration fees for applicant based outside the greater Montreal area.How to apply for a fellowship:

Cancellation policy
All cancellations are subject to a CDN $200.00 per registrant processing fee if done before May 15. Cancellations received before June 15 are subject to a CDN $400.00 per registrant processing fee.Cancellations submitted after June 15 will not be qualified for refund or credit. To receive cancellation credits, SSM4 must be notified of the cancellation in writing. Substitution of delegate/s with the member/s of the same organization is permitted at any time with no penalty.
SSM4 organization reserves the right to cancel the Summer School on Medicines 2012. If the program is cancelled, SSM4's liability is limited to a full refund of the program fees already paid.

Participation
A certificate of participation will be awarded by the Faculty of Pharmacy of the Université de Montréal to participants who complete the program to the satisfaction of the institution.
Responsabilities
The SSM4 organizers cannot be held responsible for the travel to and from Montréal and for lodging arrangements before, during and after the SSM4.
